You will receive a receipt for any payments or co-payments that you make at the time of your appointment. You can request full payment details from your insurance provider.

Why am I getting bills from University Rehabilitation?

​In the event that your insurance company denies your claim for any reason or fails to make a timely payment to University rehabilitation for services rendered to you, you are responsible for paying the balance due.

 



I received a notice that my insurance company has paid on my bill - but I can't understand how they calculated their payment amount. Do you know?​​

Each Insurance company we support has negotiated a predetermined rate with us. This rate is honored by us when submitting a claim to the insurance companies.

Obtaining a Copy of Your Medical Records

We require a written request/authorization for release of health information for release of medical records.

 

The release of protected health information is governed under Federal and State Regulations. 

​

A valid photo ID or Durable Power of Attorney is required.

​

The faxing of medical records to non-health professionals is prohibited.

​

Please allow 5-7 business days for us to process your request.

​

The fee for copies is $1.00 for up to 25 pages, then $0.25 cents for each additional page.

Insurance

We accept Medicare, Medicare Private Fee for Service Replacement Plans, most all private insurances such as Humana, United Health Care, Cigna and some Blue Cross Blue Shield Plans.  We will provide a more detailed list of accepted insurances at your request for our services.
